The five levels
Every patient starts with an assessment, then moves up level by level.

1. Pain relief
At the first level the aim is pain relief. Manual therapy here is more than pain relief: with joint mobilisation and sensory input it opens a window for relearning healthy movement, which active physiotherapy then builds on. The clinic’s protocol is a block of 12 sessions, twice a week in the painful phase, with roughly equal parts manual therapy and physiotherapy, often supported by physical therapy (for example ultrasound, TENS or kinesiology taping).

2. Restoring joint range of motion
With mobilisation and active exercise, the joints regain the range of motion that everyday movement needs.

3. Correcting muscle weakness
Targeted exercise builds strength and muscle activation. If the problem is not a lack of strength but the wrong activation, we first relearn switching the muscle on at the right time.

4. Building functional stability
Core and segmental stability, movement control, and practising the movements of daily life, work or sport. Posture is treated here as something your body regulates all the time, not as a static position.

5. Return to sport or full activity
With gradually increasing load you return to work, sport or a higher level of activity. The maintenance programme, Medical Fitness, then helps you keep and build on what you have achieved: in a clinical setting, led by a healthcare professional who knows your history.

Why 12 sessions at level 1?
Under MozgásKlinika’s protocol, 12 sessions are recommended at level 1, twice a week, with roughly equal parts manual therapy and physiotherapy. The reason: tissue adaptation and the nervous system’s motor learning need enough stimulus, regularly enough, which occasional sessions cannot give. The programme is therefore planned in blocks of 12 sessions, ideally within about three months: twice a week in the painful early phase, then less often as your condition allows.
Why not stop as soon as the pain eases?
In MozgásKlinika’s experience, level 1 has the highest drop-out rate: many patients stop once the acute pain eases. The clinic considers this a mistake, moving up a level requires the limitations of the previous level to be resolved well enough, and if function and load tolerance are not rebuilt, the risk of relapse grows. The number of sessions can never be guaranteed in advance: it depends on individual response and the professional assessment.
Who created the MozgásPiramis®?
The MozgásPiramis® was created by Rita Szentiványi, professional lead of the MozgásKlinika franchise network. She earned her BSc in physiotherapy (2006) and her manual therapy qualification (2008) at Semmelweis University, and her MSc in physiotherapy at the University of Pécs (2017).
